In East Renfrewshire, the Community Health and Care Partnership (CHCP) is being formed bringing together local health care services and East Renfrewshire Council-run social work services. The CHCP will lead the health and social care aspects of community planning and be responsible for ensuring public participation. This paper sets out the priorities and areas for action identified by the wider health improvement workforce within East Renfrewshire. These actions were primarily identified through a Health Improvement Development Event described below and a subsequent range of meetings with additional stakeholders and the Health Improvement Planning Group.Collections
