Description
NHS GGC commissioned a programme of research to explore the experiences of carers in acute hospital settings, determine the suitability of the approach taken to include carers in the discharge planning process and use this evidence to inform any changes to practice required to make the Carer Pathway more effective in helping the Health Board meet their requirements under the Act.
The research aimed to establish:
• To what extent the current approach is working
• Whether it was effective for all its stakeholders (i.e. carers, patients and health care professionals)
• What factors were supporting effective identification and involvement of carers
• How any barriers to identifying and involving carers had been addressed
• What lessons could be learned from the implementation of Pathway in hospital settings
• What improvements should be considered to ensure that the needs of carers and the people they care are met in the future
• Key recommendations for future practice in identifying and supporting carers.
